Community Engagement Coordinator 

Location: Syracuse or Watertown 

Salary: $22.00 - $25.00 Hourly 

Classification: Full-time, Non-Exempt. (35 Hours) per week; occasional evenings and weekends are expected for workshops, conferences, and seminars. 

Position Summary: This leadership role is responsible for supervising a harm reduction-focused team working with BIPOC and justice-involved people who use drugs (PWUD). The position oversees outreach, client engagement, and service delivery, ensuring alignment with harm reduction, trauma-informed, and culturally responsive principles. Key duties include supervising staff (Prevention Care Managers and Engagement Specialists), conducting quality assurance, coordinating care with external partners, and ensuring compliance with grant and agency requirements. The role also involves staff training, advocacy, and maintaining strong community linkages to reduce barriers to care.

Minimum Qualifications: 

1. Bachelor’s degree or a minimum of 2+ years’ experience working in non-profit social service agency providing direct client related services to diverse populations (e.g., people who use drugs (PWUD), justice involved individuals, mental health diagnosis, etc.). This is a critical component to the agency and the work that is done in the community. 

2. Case management and healthcare experience 

3. Excellent organizational and time management skills. 

4. Fluent in both Spanish and English preferred 

5. Computer skills and proficiency in Microsoft Office are a must; Electronic Health Records systems preferred. 

6. Sensitivity to AIDS, chronic illness, substance abuse issues and strong commitment to the mission, vision, and values of ACR health are essential. 

7. Lived Experience of homelessness, substance use, as well as BIPOC and LGBTQI+ preferred 



Primary Responsibilities: 

1. Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F and HIPAA, ACR Health policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law and HIPAA. 

2. Oversee outreach and client recruitment efforts to ensure consistent, effective engagement of the priority population, particularly BIPOC and justice-involved people who use drugs. 

3. Supervise Prevention Care Managers and Engagement Specialists, providing regular support, coaching, and case-specific consultation for complex client situations. 

4. Ensure services are delivered aligned with harm reduction principles, trauma-informed care, and culturally responsive practices. 

5. Conduct routine quality assurance (QA) reviews and lead quality improvement (QI) initiatives to strengthen service delivery and data integrity. 

6. Review and submit timely programmatic reports, track performance metrics, and ensure compliance with grant and agency requirements. 

7. Monitor outreach, engagement, and care coordination activities to ensure program fidelity and responsiveness to the needs of BIPOC and justice-involved PWUD. 

8. Coordinate with external partners, including culturally comprehensive providers, housing and medical services, and justice system entities, to support referrals and collaboration. 

9. Facilitate internal team meetings and case conferencing to promote cross-collaboration and shared decision-making. 

10. Support the onboarding, training, and professional development of staff, particularly those with lived experience. 

11. Attend all required meetings, training, and calls designated by the New York State Department of Health (NYSDOH) and represent the program in alignment with its goals and values. 

12. Ensure proactive and productive linkages with area social services providers. 

13. Authorize and arrange for necessary support, medical and stabilizing services for the client to develop and maintain effective health care. 

14. Reduce barriers to services through persistent advocacy efforts and appropriate documentation of service gaps. 

15. Advocate for thorough and responsive services acquisition and for systems development through appropriate documentation of service gaps; represent client when advocacy is necessary. 

16. Develop and maintain working knowledge of all areas of service providers and client resources; keep abreast of developments in the medical, behavioral, social service, legal and other areas related to client services provision. 

17. Complete all required staff, department, and ACR Health training, as well as a minimum of 24 hours annually of professional development training and certifications. 

18. Advocate for medical, legal, social, financial, housing, and other services, as appropriate. 

19. Provide crisis intervention when appropriate. 

20. Maintain confidentiality of all clients, peers, and volunteers 

21. Maintain working knowledge of all care management program regulations, policies and procedures, as well as required database and/or EMR documentation requirements. 

22. Attend all required and recommended staff training and in-services. 

23. Ability to pass Medicaid fraud background check. 

24. Perform other duties as directed by the Health Homes Manager, Director of Care Management Services, Chief Program Officer and/or the Chief Executive Director.
